It's an emergency if you notice: chest pain, difficulty breathing, sudden severe headache, uncontrolled bleeding, loss of consciousness, signs of stroke (facial drooping, slurred speech, limb weakness), severe allergic reactions, high fever with seizures, or major trauma. In these cases, go straight to a hospital ER, not a clinic. For non-life-threatening issues—mild fever, minor cuts, common infections, routine follow-ups—a clinic visit is appropriate. When in doubt, especially with sudden or severe symptoms, err on the side of hospital care or call emergency services (108 in India).
